CMS Adds Five Tracks to ACCESS Model for Heart Failure, COPD, SUD, Tobacco Cessation, Extended MSK
CMS is expanding the ACCESS (Advancing Chronic Care with Effective, Scalable Solutions) Model in spring 2027 to include five new tracks: heart failure, Chronic Obstructive Pulmonary Disease (COPD), substance use disorders (SUD), tobacco cessation, and extended musculoskeletal (MSK) condition support. The expansion builds on the existing ACCESS Model's approach to chronic care management through alternative payment mechanisms. The new tracks become available to participating providers and health systems starting spring 2027. For Medicaid managed care organizations and state agencies with dual-eligible populations or coordinated care models, the expansion creates new opportunities for care integration and potential shared savings arrangements in behavioral health and chronic disease management.
The SUD and tobacco cessation tracks create new federal payment pathways that Medicaid MCOs can leverage for populations with overlapping Medicare-Medicaid eligibility, particularly as states expand behavioral health integration requirements.
