Iowa Nursing Homes Top Nation in Staffing Citations for FY2026

An Iowa Capital Dispatch review of CMS data finds Iowa nursing homes received more insufficient-staffing citations than any other state in fiscal year 2026, with 87 citations across 66 facilities despite the state having far fewer nursing homes than larger states like California or Illinois. Seven homes were cited three times and seven more twice; Iowa also ranked among the top states for violations of the registered-nurse-on-duty requirement. The review notes short-staffing citations typically carry no financial penalties in Iowa unless they contribute to documented safety or neglect issues. The findings follow years of opposition from Gov. Kim Reynolds and Attorney General Brenna Bird, along with the Trump administration, to federal efforts to impose stricter minimum staffing mandates on nursing facilities.

Why it mattersPersistent understaffing without financial penalties signals ongoing quality-of-care risk for Medicaid-funded nursing facility residents and increased exposure for state oversight agencies as federal minimum-staffing rules remain contested.
