Rush's Illinois Medicaid Program Payments Nearly Double in FY2026

Rush University System for Health reported operating income of $56.6 million (1.3% margin) for fiscal 2026, up from $32.5 million (0.8% margin) in fiscal 2025, per audited financials for the year ended June 30. Total revenue grew 13.9% to $4.4 billion, driven by patient service revenue growth and a near-doubling of net benefit from the Illinois Hospital Assessment Program, which rose to $219.7 million as Medicaid payments under the program climbed to $386 million and state assessment taxes rose to $166.3 million. Rush's net income reached $293.9 million, boosted by $225.6 million in investment income. The results come as Rush and Evanston-based Endeavor Health plan to merge by early 2027, creating a 12-hospital system under the Rush brand with Endeavor funding a $1 billion investment.

Why it mattersThe sharp increase in supplemental Medicaid payments through Illinois's hospital assessment program underscores how heavily hospital margins now depend on state-directed Medicaid financing mechanisms, a dynamic other state Medicaid agencies and CMS are scrutinizing for sustainability and federal matching exposure.
