Dartmouth Health Launches In-Home Care Program With $20M Gift

Dartmouth Health and the Geisel School of Medicine are launching Dartmouth Health at Home, a new in-home primary and palliative care program funded by a $20 million donation from alumnus Richard Levy. The program will pilot in January 2027 serving 10 to 20 patients, with plans to expand across New Hampshire and Vermont to serve over 1,000 patients annually using roughly 30 clinicians. It will deploy interdisciplinary care teams and establish a Family Caregiver Institute, while the Dartmouth Institute for Health Policy & Clinical Practice studies cost and outcome impacts to help ensure long-term financial sustainability. The initiative aims to reduce hospitalizations and emergency visits by catching medical issues earlier in rural, hard-to-reach populations.

Why it mattersHealth systems and payers tracking home-based care models will watch whether Dartmouth's approach can demonstrate cost savings and sustainable financing that could inform value-based arrangements with Medicaid managed care plans serving rural, dual-eligible, and LTSS populations.
