CMS Renames Hospice Item Set System to HOPE, Adds Real-Time Data Collection

CMS is modifying its existing Hospice Item Set (HIS) System of Records, renaming it the Hospice Outcomes and Patient Evaluation (HOPE) system. The change adds real-time data collection at the time of patient assessments to improve understanding of care needs and coordination. The system collects standardized hospice patient data for quality measurement, regulatory compliance, reporting, research, and policy functions.
