57 Hospitals Have Converted to Rural Emergency Status Since 2023

Becker's Hospital Review compiles CMS data showing 57 hospitals across 22 states have converted to Rural Emergency Hospital status since the designation took effect in January 2023. The list breaks down conversions by state, with Mississippi, Oklahoma, Texas and Arkansas each seeing six or more conversions. The designation allows eligible rural facilities to drop inpatient beds in exchange for enhanced Medicare outpatient reimbursement and a facility payment, aimed at helping financially distressed rural hospitals keep emergency and outpatient services open. The report also notes two facilities in the list, in Michigan and Nebraska, that have since closed or filed for bankruptcy despite converting.

Why it mattersState Medicaid agencies and rural providers need to track which facilities have shifted to outpatient-only emergency status, since the conversion changes covered service lines, provider billing categories, and network adequacy calculations for rural Medicaid enrollees.
