Study Identifies Patient Traits Linked to Stronger GLP-1 Response

An Oct. 13 analysis from the Health Transformation Alliance examined linked claims and EHR data for 12.7 million patients from 2018 to 2025 to identify which patients respond best and worst to GLP-1 therapy. Among diabetes patients, the strongest responders had higher baseline HbA1c, lower overall illness severity, and no recent ED visits or hospitalizations, while weaker responders had higher illness severity and recent acute care use. Among obesity patients without diabetes, older patients in a mid-range weight band with fewer comorbidity-related prescriptions and no sleep disorder saw the greatest weight loss. The authors caution the study is observational, does not predict individual outcomes, did not assess workforce productivity, and reflects only about a year of follow-up, leaving long-term ROI uncertain.

Why it mattersHealth plans and employers designing GLP-1 coverage criteria and utilization management policies need patient-level response data to target benefit design, even though the authors warn against using these findings to set coverage criteria.
