Oklahoma Reports Medicaid Improper Payment Rate Far Below National Average

The Oklahoma Health Care Authority reports its Medicaid (SoonerCare) improper payment rate was 0.15% in 2025, far below the 6.12% national average, with CHIP at 0.88% versus a 7.05% national average. The agency says the rates, reviewed every three years, reflect staff quality control, enhanced data integrity, provider credentialing and revalidation, post-payment audits, and data-sharing with partners like Equifax. Oklahoma ranked among the lowest-error states in both the 2022 and 2025 reviews, even after transitioning to managed care. The report comes as Attorney General Gentner Drummond has requested an audit of the agency's managed care oversight, and as the state separately faces a federal deadline to cut its SNAP error rate below 6% or face increased administrative costs.

Why it mattersLow improper payment rates strengthen Oklahoma's position amid a pending state audit of its managed care oversight and ongoing federal scrutiny of program integrity performance.
