Medicaid Cuts, Rural Health Fund Dominate Maine Senate Race

Maine's U.S. Senate race between Republican incumbent Susan Collins and Democratic challenger Troy Jackson has become a referendum on the "One Big Beautiful Bill," the federal spending package expected to cut Medicaid funding nationally and in Maine. Collins voted to advance the bill but against final passage, and now points to the $50 billion Rural Health Transformation Program she authored, which awarded Maine $190 million in fiscal year 2026 funding now fully committed to hospital sustainability, workforce, and technology projects. Jackson and allied groups argue Collins enabled the cuts by voting to advance the legislation, while KFF analysts say the rural fund will mitigate but not offset the law's larger Medicaid reductions. A dispute also emerged over Collins' claim that Maine nearly lost the funding due to compliance issues, which state health officials dispute.

Why it mattersMaine Medicaid stakeholders, including rural hospitals and birthing centers already facing closures, face a funding gap between new transformation grants and deeper federal Medicaid cuts that the political debate is obscuring.
