Iowa Tops Nation in Nursing Home Staffing Citations, Analysis Finds

An Iowa Capital Dispatch analysis of CMS data, reported by Skilled Nursing News, finds Iowa leads the nation in nursing home citations tied to insufficient staffing despite having far fewer facilities than larger states. Iowa recorded 87 staffing citations across 393 facilities, ahead of Illinois (85 citations, 669 facilities) and California (59 citations, 1,169 facilities), with Michigan, Pennsylvania, New York, Ohio, Missouri, Minnesota and Wisconsin rounding out the top ten. The analysis notes staffing violations carry no financial penalties in several states including Iowa, even as short-staffing contributes to documented neglect, including residents waiting 39 to 90 minutes for call light responses. A separate 2025 report from the Long-Term Care Community Coalition found nine in ten nursing homes nationally fell below expected staffing levels, understaffed by an average of 24% daily.

Why it mattersState Medicaid agencies and nursing facility operators face mounting scrutiny over staffing compliance that, absent financial penalties in many states, has done little to curb documented resident safety lapses funded largely through Medicaid.
