Ennoble Care Expands Home-Based Primary Care Into Five States

Ennoble Care, a Hackensack, New Jersey-based home-based primary care company, expanded into Florida, Kentucky, Ohio, Mississippi and Texas in September, bringing its footprint to 16 states. CEO Kush Das said the growth targets areas with large, aging populations facing primary care deserts, combining contiguous geographic expansion with opportunistic practice acquisitions. The company plans to enter roughly 10 more states over the next year and is also exploring a narrowly tailored Medicare Advantage chronic special needs plan, alongside a planned shift from the ACO REACH model to MSSP and LEAD models as ACO REACH sunsets at the end of 2026. Das flagged health plan network credentialing, particularly payers' failure to distinguish home-based from office-based primary care in network adequacy assessments, as the biggest operational barrier to expansion.

Why it mattersHealth plans and state Medicaid and Medicare Advantage networks will face growing pressure to credential home-based primary care providers distinctly from office-based practices as companies like Ennoble scale into new markets.
