Governor Races Could Shape Rollout of Medicaid Work Requirements

Becker's Hospital Review reports that 30 of the 44 states required to implement Medicaid work requirements in 2027 are holding gubernatorial elections Nov. 3, with winners taking office just as HR 1's 80-hour monthly work rule takes effect for most expansion adults. Governors cannot waive the federal mandate but control implementation details like compliance verification, medically frail exemptions, and whether to seek more time from CMS. Races in Georgia, Pennsylvania, Oregon and Wisconsin illustrate divergent candidate approaches, from opposing work requirements entirely to pledging stricter anti-fraud enforcement and independent audits. Five governors have already asked HHS to delay the Jan. 1 deadline, and 25 states plus D.C. are suing CMS over narrowed medically frail exemptions, with a federal ruling expected by year's end.

Why it mattersState Medicaid agencies face incoming leadership transitions that could reshape verification systems, exemption criteria, and enforcement timelines just as work requirements and coverage-loss risk begin in 2027-2028.
