Centene's headcount fell 11.8% from 61,100 at the end of 2025 to 53,900 in the third quarter of 2026, driven by an enterprise optimization push and contract exits. The company's Oct. 27 earnings report shows $276 million in severance costs and $35 million in third-party vendor costs tied to the restructuring, alongside continued executive turnover at the state and corporate level. Centene posted a $71 million net loss for the quarter, a sharp improvement from a $6.6 billion loss a year earlier, and raised its 2026 earnings and revenue guidance. Medicaid membership dropped 6.4% year over year to 11.9 million, while marketplace membership fell 41.6% as the company continues to shed exchange enrollees.
Why it mattersState Medicaid agencies and providers contracting with Centene should watch for continued network and staffing disruptions as the company shrinks headcount and sheds membership across Medicaid and marketplace lines.
