Advocacy Group Finds 75 Iowa Nursing Homes Get Lighter CMS Inspections

The National Consumer Voice for Quality Long-Term Care analyzed federal data and found 75 of Iowa's 393 nursing facilities now qualify for CMS's new Risk-Based Survey process, a shorter, less intensive inspection launched September 8, 2026, for homes deemed "lower risk." Qualifying facilities display a "gold trophy" icon on CMS's Care Compare site and face fewer resident interviews and fewer state inspectors reviewing their care. The analysis found some flagged homes, including Waterloo's Friendship Village, had recent citations for insufficient staffing, infection control failures, and resident harm, with Friendship Village fined $5,000 and hit with a Medicaid-Medicare payment suspension for new admissions the same month it earned gold trophy status. Nationally only 12% of nursing homes qualify for the abbreviated process, which CMS says frees inspector resources to focus on problem facilities.

Why it mattersState Medicaid agencies and CMS face scrutiny over whether the new risk-based survey criteria adequately screen out facilities with recent harm citations before reducing oversight of homes serving Medicaid-funded nursing facility residents.
