No managed care organizations operate in Alaska Medicaid; the program is fee-for-service statewide, confirmed by CMS's own national managed care enrollment dataset showing zero managed care plans for Alaska. In place of an MCO structure, DHCS runs AMCCI, a voluntary program that assigns members a state case manager for care coordination without changing how claims are paid.

Long-term services and supports are fee-for-service, administered by the Division of Senior and Disabilities Services (SDS) rather than through any health plan. Five 1915(c) HCBS waivers currently serve distinct populations: Adults with Physical and Developmental Disabilities, Alaskans Living Independently (ages 65+ or with physical disabilities ages 21-64), Children with Complex Medical Conditions, the Individualized Supports Waiver, and the People with Intellectual and Developmental Disabilities Waiver. A sixth waiver, for Psychiatric Residential Treatment Facilities, has been terminated. Community First Choice (CFC), a Section 1915(k) State Plan option, is a separate state plan HCBS benefit alongside these waivers, not folded into any of them.

Not integrated into managed care

Behavioral health is not carved out to a separate vendor; like every other Medicaid service in Alaska, it's paid fee-for-service directly to providers, not administered through a plan. What does set it apart is federal authority: standard State Plan behavioral health benefits are supplemented by the Behavioral Health Reform 1115 demonstration, administered by the Division of Behavioral Health, which extends coverage for substance use disorder treatment and additional behavioral health services beyond the base State Plan benefit. DBH administers this demonstration as its own division, separate from DHCS's general Medicaid program administration.

Carved in

Dental is a direct fee-for-service Medicaid benefit administered by the Department of Health itself. Alaska does not contract with a third-party dental benefit administrator, a departure from the vendor-administered dental model most other states use.

Magellan Rx Management operates Alaska Medicaid's Drug Lookup tool and hosts the state's Preferred Drug List, developed by the Pharmacy and Therapeutics (P&T) Committee. This is a different vendor from OptumRx, which administers the pharmacy benefit for the separate AlaskaCare state-employee health plan (a different agency, not Alaska Medicaid) through the Division of Retirement and Benefits.

Alaska is one of a small number of states that run non-emergency medical transportation without a broker. Requests go through a statewide call center operated by an administrative services contractor under DHCS's direction; DHCS certifies transportation providers directly, and the state pays them fee-for-service. Given Alaska's geography, a meaningful share of NEMT spending goes to air and ferry travel rather than ground transportation, a pattern uncommon among other states' NEMT programs.

Conduent operates Alaska Medicaid's claims processing system and the public Health Enterprise Portal (medicaidalaska.com). HMS holds a separate, narrower contract for third-party liability coordination of benefits and audit recovery, identifying and recovering Medicaid expenditures owed by other liable payers: a distinct role from general claims processing, not a fiscal-agent handoff from Conduent.
